Outreach services

Definition & Purpose

Outreach workers are knowledgeable about many resources and programs available to seniors.  Appointments for telephone consultations are available.

Eligibility

Many programs are available to all persons age 60 an older, while some do have income and/or asset limitations and other guidelines for eligibility.

Scheduling

Please call the office at 419-586-1644 to schedule a consultation.  

Available programs include but are not limited to:

Social Security - Low income subsidy & supplemental security income
Golden Buckeye
Weatherization
Health Insurance claim form assistance
Medicare drug card assistance
PASSPORT
Care Coordination
Lifeline
Elderly legal services
Living Wills & Durable Power of Attorney
Voter Registration
Financial assistance for health related needs & utilities
Loan program for wheelchairs, walkers, canes, toilet seats and bath chairs
